When Jaw Pain Becomes a Dental Emergency

Jaw pain is a common complaint — millions of Americans experience some degree of temporomandibular joint (TMJ) soreness. Most of the time, this is a chronic, manageable condition. But there are situations where jaw pain crosses into a genuine dental emergency: when the jaw locks shut or open, when acute joint injury occurs, when infection involves the jaw joint, or when jaw pain is actually a dental emergency masquerading as joint pain.

Common TMJ Symptoms (Usually Not Emergencies)

The following are common, chronic TMJ symptoms that warrant treatment but are generally not emergency situations: • Mild to moderate jaw soreness, especially in the morning • Clicking, popping, or crunching sounds in the joint when opening or closing • Limited mouth opening that is gradual and has been present for weeks to months • Headaches arising from jaw muscle tension • Ear pain attributed to TMJ • Facial pain from muscle soreness without acute trigger

These conditions deserve evaluation and treatment — but an appointment within a few days is typically adequate.

TMJ Situations That Are Dental Emergencies

LOCKED JAW (CLOSED LOCK): The articular disc in the TMJ becomes displaced in a way that physically blocks the jaw from opening. A patient with closed lock typically woke up unable to open their mouth more than a centimeter or two. Attempting to open further causes immediate, sharp pain. Same-day evaluation is appropriate.

LOCKED JAW (OPEN LOCK / DISLOCATION): The lower jaw slides too far forward and gets "stuck" in the open position. The patient cannot close their mouth. This is a medical/dental emergency requiring manual reduction. Go to an emergency room or call us immediately.

ACUTE TMJ TRAUMA: A direct blow to the jaw from a punch, fall, car accident, or sports injury can cause acute injury including bleeding into the joint, disc displacement, or condylar fracture. Same-day evaluation and imaging required.

JAW PAIN THAT IS ACTUALLY TOOTH-RELATED: Many patients who come to us with "jaw pain" are actually experiencing referred pain from a dental abscess, impacted wisdom tooth, or severe pulpitis. Lower back molars in particular refer powerfully to the jaw and ear, mimicking TMJ problems perfectly. A thorough dental examination quickly differentiates true TMJ pain from dentally-referred pain.

Home Management of Mild TMJ Flare-Ups

Soft diet: Avoid hard, chewy, and crunchy foods
Ibuprofen: Anti-inflammatory effect is particularly valuable for TMJ inflammation
Warm compresses: Apply to jaw area for 15–20 minutes several times per day to relax muscle spasm
Avoid wide opening: Don't yawn excessively; avoid foods requiring very wide opening
Nightguard: If you have a pre-existing nightguard, wear it consistently
Stress reduction: Clenching and grinding increase dramatically with stress
